| "First Day Forms" for Math Class! 2010-2011 School Year For All Mr. Cantlin's Math Classes |

There are some forms I would like my math students and parents to fill out at the beginning of each school year. If you have initiative you could print these out and bring the completed forms to the first class on Thursday, August 12th. Or, if you are an overachiever with initiative and a sense of urgency...I included the original Microsoft Word copies of these forms so you could download, fill out on your computer, and email the completed forms to me now. Form 1 of 2 - Introduction Letter and Math Help & Redo Policy Students and Parents read, parents sign and return. Parents, if you fill this out online just type your name on the signature line and email the form from your email address, the reason I ask to have it signed is some students (a very few) don't like their parents to know extra help if available - they are worried they might have to work :-).
